What’s Your Move Barnsley is about helping people to be more active for good physical and mental health. This document for healthcare professionals provides a snapshot of health benefits to being active inclusive of immediate and long-term benefits and ways of incorporating physical activity into daily lifestyle. Barnsley Age UK Older People Physical Activity Alliance are an organisation aiming to increase the provision of physical activity programmes across Barnsley.

Our activity finder is a tool accessible across Calderdale and Kirklees to support people and workforce in identifying activities local to your home.

Active Calderdale aims to tackle inactivity in Calderdale by making it an embedded part of day-to-day life for everyone in the borough. Get Doncaster Moving signposting opportunities to find activities near you, linked to GDM is Your life Doncaster which provides information for adults, young people and children to live happy, healthy lives and discover what is available in their local communities.

Sheffield City Council operate 'Keeping well in your local community' and Sheffield Clinical Commissioning group have developed a tool to support clinicians on guiding patients to activities that can support living a healthier lifestyle (see appendix 1).

The prehab4cancer and recovery programme is a free exercise, nutrition and wellbeing scheme, designed to help people with a recent diagnosis of cancer prepare for, and cope better with their treatment.  This case study has been shared to illustrate an example of how Physical Activity can play a huge part in recovery and reduction of further problems and aims to help people take an active role in their care and live as well as possible with and beyond cancer.

Summary of Evaluation

  • Greater Manchester (GM) Cancer was the first regional system in the UK to introduce a large scale Pre- habilitation and rehabilitation programme as a standard of care for cancer patients. It started as a two-year transformation project, launched in April 2019, aiming to provide patients with the best opportunity for excellent quality outcomes and long-term survival.
  • The programme includes physical exercise, nutritional screening and advice, and emotional wellbeing support both before, during and after cancer treatment. Aiming to improve quality of life, reduce length of stay and enhance recovery, and categorised interventions into universal, targeted and specialist

Headline Evaluation Points

Patients are optimised prior to surgery and have long-lasting health benefits following post-operative rehabilitation. This reduces demands on healthcare services throughout the cancer pathway.

  • Quality of life and physical activity improvements indicate long-term behaviour change and health improvement, with patients taking control of their care.
  • Improvements are seen in both ward and critical care bed day usage resulting in improved elective care capacity and effective use of resources. Additional positive impacts on 30 and 90-day readmission and emergency department admissions have been observed.
  • Efficiency improvements to pathways are visible which support delivery of elective care and cancer recovery plans, and achievement of cancer performance standards.
  • Evidence that supports improved survival in patients who complete prehab.

Headline results include

  • 5-day reduction in hospital length of stay per prehab patient
  • 4-day reduction in critical care length of stay per prehab
  • 550 ward bed days ‘released’
  • 146 critical care bed days ‘released’
  • Bed days ‘released’ from 1000 colorectal prehab patients enable 179 additional patients to access timely surgical pathways.

Theories of behaviour change are essential to understand Physical Activity and provide an organising framework for effective intervention. When helping people to move more we have to consider our approach in changing behaviour around habits, lifestyle and wider determinants. To support transition and ensure positive change to lifestyle. We have provided some examples of behaviour change theories to support you.

Transtheoretical Model 

This relates to a model of behaviour change that assesses an individual’s readiness to act on a new healthier behaviour and provides strategies or processes to guide the individual through the stages of change.

COM-B 

The COM-B model of behaviour is widely used to identify what needs to change in order for a behaviour change intervention to be effective. It identifies three factors that need to be present for any behaviour to occur: capability, opportunity and motivation. Capability refers to an individual’s psychological and physical ability to participate in an activity. Opportunity refers to external factors that make a behaviour possible. Lastly, motivation refers to the conscious and unconscious cognitive processes that direct and inspire behaviour.
